Requested Agenda Item Description: The Salt Lake County Board of Health currently has one vacancy with the completion of Board terms for the following:
?Dr. William Cosgrove, physician licensed in Utah
In addition, we are requesting to have Board Chair, Dr. Dorothea Verbrugge, upcoming vacancy filled, as she will end her term with the Board on December 31, 2021. The following are recommendations for Board nominees that will fulfill qualifications as outlined in Board bylaws:

Physician licensed in Utah:
Stacey Bank, MD - is the Medical Director at the University of Utah Intensive Outpatient Clinic in Salt Lake County and lives in Salt Lake City, Council District #4. Dr. Bank oversees an integrated clinic for high-risk patients with both complex medical and behavioral health needs and works closely with the population health services at the University of Utah and is passionate about identifying and treating health issues that affect specific members of the community, as well as the community at large. She has been recommended by both Karen Crompton, Human Services Director, and Deputy Mayor Erin Litvack. This appointment would be for one 3-year term from July 1, 2021 - June 30, 2024.

Kencee Graves, MD - is the Associate Chief Medical Officer and Associate Professor at the University of Utah Health in Salt Lake County and lives in Millcreek, Council District #4. Dr. Graves has work closely in the past on developing plans that will keep our community, children, and health care work force safe and healthy and has served on the Fourth Street Clinic Board since 2019. She often sees patients that don't have the ability to advocate for themselves that are diverse in age, gender, race, ethnicity, illness, and life experiences. She has been recommended by Mitchell Park, Council District Attorney....
